File format detection

Tracked in kothar/csview2-macos#113.

Sample the start of the file before indexing to detect the line terminator, delimiter and unbalanced quotes, and use the result as the initial file format. If indexing still reads a long way without completing a row, warn and suggest the likely fix rather than continuing silently.
