Detect file changes

Detect if the underlying file has changed and re-index. Since we need to parse the file on the fly, any changes to the underlying file are likely to corrupt the parsing.

Implementation Plan

Display a warning, but don't trigger re-indexing if the user isn't interacting with the grid to avoid constant re-indexing.

If the file has been appended to we only need to extend the index from the last block. To detect this we should keep a checksum of some lines/blocks or check if the start/end delimiters match what we saw during indexing. If something has changed in the middle of a block that doesn't affect the delimiter starting position, it might be possible to repair just that block? Otherwise everything afterwards needs to be re-indexed. Initial implementation should just be to re-index.
